Important Information
Location: All office visits take place at Soul Wellness, 500 S Main Street, Mooresville, NC 28115. This is our designated office space — you do not need to live in Mooresville to book an appointment.
Appointment Duration: Plan for 60–90 minutes for your office visit, which includes a comprehensive health assessment and consultation.
Weekend Availability: Friday, Saturday, and Sunday appointments are available by request only. Please contact us to confirm availability before booking a weekend time.
Minors: A Parent/Guardian MUST be present for any appointment with a minor. It CANNOT be a sibling, even if 18 years of age.
Additional Fees: Additional fees may apply during the time of appointment for specific procedures or extended consultation time.

How to Complete Your Booking

Choose whether you are a new or returning patient above, then fill out the required fields to provide us with the information we need to prepare for your office visit.
Submit Your Payment
Use our secure payment system to pay the $165 office visit fee. If you are interested in becoming a member and receiving the reduced rate of $125, view our membership options.
Access the Scheduling Page
After your payment is processed, you’ll be redirected to a private scheduling page to select your appointment day and time. Regular availability is Tuesday and Thursday, 2:00–3:30 PM at Soul Wellness in Mooresville, NC.

Finalize Your Appointment
Once booked, our provider will review your information and confirm your appointment. You will meet at Soul Wellness, 500 S Main Street, Mooresville, NC 28115 at your chosen time slot for your comprehensive office visit.
